Overview

The Rural Health Transformation Program (RHTP), established through the One Big Beautiful Bill Act (OBBBA), provides $50 billion over five years to help states strengthen and modernize rural health systems.

Administered by CMS, the program requires states to develop and implement transformation plans focused on access, sustainability, and innovation.

RHTP presents a unique opportunity for states to move beyond short-term funding and implement lasting system change through well-designed payment models and coordinated implementation strategies.
